German qualifier Oscar Otte's incredible US Open campaign has come to a painfully sad end after a gutsy four-set defeat against Italy's Matteo Berrettini.

A devastating injury wrecked the World No.144's hopes of causing a huge upset against sixth seed Berrettini, who booked a quarter-final berth with a 6-4, 3-6, 6-3, 6-2 win.

It was an incident-packed match for Otte, who bravely fought on through a wrist injury that was causing him obvious discomfort.

Rather than retire hurt, the German stuck it out to the end to earn the respect of the crowd and his opponent - who encouraged fans to applaud him while serving for the match.

Otte's hopes were dashed when he suffered a wrist injury at 1-2 down in the fourth set after stumbling awkwardly after an overhead smash.

The German somersaulted backwards and crashed down heavily on his wrist, before calling for a medical timeout and receiving treatment for the issue.

Incredibly, Otte managed to hold serve after the medical timeout, but failed to win another game in the rest of the match.

The 28-year-old also had a crazy mishap in the first set, where he raced to retrieve a Berrettini shot before toppling over the net onto the Italian's side of the court.

The "brutal" drama left tennis fans devastated for the courageous German.

Matteo Berrettini into last eight at US Open again

Berrettini's win sees him return to the last eight at Flushing Meadows for the second time in three years, despite having to overcome a second set wobble.

As well as his classy gesture for Otte at the end of the match, Berrettini was full of praise for his opponent afterwards as well.

“We can talk about the first part of the match and he got injured, and there wasn’t a match anymore,” Berrettini said in his on-court interview.

“I said to him that he had a great run and he didn’t deserve to end like this. Now it’s tough to talk about the match, but I really enjoyed the support from the beginning.

"There are a lot of people that really support me. Let’s talk about the atmosphere that was really great.”

Joining Berrettini in the quarter-finals is German Alexander Zverev, beat Jannik Sinner 6-4 6-4 7-6 (9-7) to move a step closer to a maiden slam title.

The Tokyo Olympics gold medallist extended his winning run to 15 matches after also winning the Cincinnati Masters title last month.

"I'm happy where I am, I'm happy with how things are, and I'm happy with how things were the last few months," the fourth-seeded German said.

"I'm in the quarter-finals now, and from here on, the matches will definitely not get easier."

Zverev's next match is against one of the tournament's many surprise packages in South African Lloyd Harris, who reached a first grand slam quarter-final with a 6-7 (6-8) 6-4 6-1 6-3 victory over 22nd-seeded Reilly Opelka.

The in-form Harris, who dispatched seventh-seeded Denis Shapovalov in straight sets in the previous round, won an incredible 92 per cent of his first-serve points, smashing 36 aces and 62 winners to seize the initiative against Opelka.
